Information from your visit
The hosting service may process standard request information such as IP address, time, requested URL, browser information and errors for delivery, security and troubleshooting. Emailing support shares the address and information you include with our email service and support personnel.
Do not send Medicare numbers, Social Security numbers, medical records, payment details or passwords to support. The public educational pages do not require an account or a health questionnaire.
The optional checklist
The comparison checklist works in page memory by default. If you select “Save checkmarks on this device,” it uses browser local storage to remember completed steps. It stores completion indicators, not medications, diagnoses or coverage details. Uncheck that option or use Reset to remove its saved data. Browser storage controls can also remove it.
The checklist is not submitted to our server. Printing or downloading produces a copy you control.
Call tracking and referral information
Campaign-specific call pages use Ringba to assign a call number and associate referral details with calls. When a visitor starts a lookup on those pages, the submitted age, ZIP code, click identifier, affiliate and available campaign parameters may be shared with Ringba. Referral details may be retained in browser session storage for a limited attribution window.
When someone calls, the call provider receives call information, which can include caller ID, the dialed number, time and duration. The campaign’s configured events can send click-linked call or conversion details to Everflow. Recording, if used by a call recipient, is subject to its notices and practices; this website does not itself record calls.
Referrals may lead to insurance sales representatives and may result in compensation. The public homepage, guides, journal and checklist do not load Ringba or Everflow tracking scripts.
Privacy signals and choices
The campaign call-tracking integration checks Global Privacy Control, Do Not Track and an explicit measurement opt-out. An opt-out can be requested with tracking_opt_out=1 in the page URL; the preference is saved in that browser. These signals suppress referral storage and Ringba loading in the integration. Local benefit lookup remains available, while tracked calling is disabled.
Without an applicable opt-out, measurement on those pages starts automatically with the lookup. No visitor consent record is fabricated. Browser or network settings may also affect third-party behavior; we do not promise that any technical control blocks every possible collection by every service.
You can request help with access, deletion, correction or an opt-out by emailing support@planmatch.us. Explain the request without sending sensitive identity documents. We may need proportionate information to verify and fulfill it under applicable law.
Use, sharing and retention
Information is used to operate and secure the website, respond to inquiries, troubleshoot errors and, where call features are used, measure referrals and connect calls. Hosting, email and measurement providers process information for those functions. Disclosure may also occur where required by law or necessary to address misuse or protect rights.
Advertising-related or referral sharing may be treated as a sale, sharing or targeted advertising under some laws. Applicable opt-outs and browser signals are respected by the integration described above. We do not publish your support correspondence or checklist selections.
Support messages and operational records are retained as needed for their purpose, security, dispute handling and legal obligations. Provider retention varies; we do not promise a universal retention period. No online transmission or storage method is guaranteed secure.
Other important points
The website is intended for adults and is not directed to children under 13. Contact us if you believe a child has provided personal information. External websites have their own privacy practices. Processing may occur in the United States and other locations where service providers operate.
